  • Audited Financial statements for the last three (3) consecutive years
  • Annual Income Tax Returned (SIR Form 1702)
  • For NEW Companies: Please provide Financial Forecast for the next 3 consecutive years (i.e. Balance Sheet and Income Statement)
  • Other financial documents applicable to the supplier
  • Three (3) months Bank Statement

 

  1. LABOR-RELATED DOCUMENTS
  • Certification of SSS coverage
  • Certification of PhilHealth contributions
  • Certification of registration to the Home Development Mutual Fund (HDMF)
  • DOLE Clearance that company has no pending case
  • Other labor-related documents applicable to the supplier
